report an Accident, injury or near miss
An incident report is a formal recording of the facts related to an accident, injury, or near miss during your student group activities. Its primary purpose is to uncover the circumstances and conditions that led to the event in order to prevent future incidents.
If you have any accident, injury or near misses, you must report them to LSESU via the form below and email to su.societies or su.sport@lse.ac.uk.
