We encourage anyone who has experienced harassment, discrimination or bullying by any LSE student while on LSESU-managed premises, at a Union-organised event or an event organised by a LSESU society, club or representative association to share this information with us.

Confidentiality: The information you provide will be kept confidential and stored securely is only shared on a need-to-know basis and will not be disclosed to any third parties without your consent unless there are legitimate reasons requiring us to do so including Safeguarding concerns.

Hearing back from us: If you provide your contact details in the report someone will contact you as soon as possible to set-up a meeting to discuss further.

Incident/Near Miss

It is vital that all accidents and near misses are reported to LSESU.

An incident report form is a formal recording of the facts related to an accident, injury, or near miss during your student group activities.
A near miss is a potential incident that has not resulted in personal injury or damage. Examples include, unsafe environments, use of broken equipment and slips and trips hazards.
Its primary purpose is to uncover the circumstances and conditions that led to the event in order to prevent future incidents.

In all accidents, injuries or near miss cases, please aim for this report to be completed within 24 hours of the incident/ near miss. If 24 hours has passed, please still fill in this form.
